This is what your journey with us looks like

Eight steps, in plain English. You can stop, change your mind, or ask us something at any point.

  1. 01

    You hear about us

    • Usually through Adult Social Care, Livewell or SORT.
    • Sometimes through social prescribing, a leaflet, our Facebook page, or someone you know.
  2. 02

    You get in touch

    • Facebook message, email or a phone call — whichever is easiest for you.
  3. 03

    We arrange a meeting to get to know each other

    • Usually at your house, or wherever you'd rather meet.
    • We talk through what we do in more detail and how we can help you live your best life.

  4. 04

    Onboarding begins

    • Gemma or Deborah look after your onboarding.
    • We confirm your visit dates and times — Stella, our rota coordinator, sorts these.
    • You tell us your preferences on staff, and we write them down.

  5. 05

    First visits, with an introduction

    • Your first visit is an introduction to the staff supporting you.
    • Your core workers get identified during these early visits.
  6. 06

    Relationships get built

    • Over a few weeks you get to know your staff.
    • Any kinks get worked out — tell us and we sort it.
  7. 07

    A review after your first two weeks of visits

    • After your first two weeks of visits we sit down with you and review how it is going.
    • We check whether anything else has come up that you need, and how you have been getting on with your staff so far.
  8. 08

    Then a review at three months, and every year after that

    • After that first review there is no further review until three months after you started.
    • From then on we review annually with your client manager. Any concerns or queries are actioned as necessary — if your needs change sooner, the review happens sooner.

What does this mean?

  • Nothing is decided at the first meeting — it is a chat.
  • You tell us your preferences on staff and we write them down.
  • If something is not working, tell us and we sort it.
  • Anything about visit times or the rota goes to Stella, our rota coordinator.
